Service Coordination
You deserve to know what's expected of you and to have your strengths valued. Our staff coordinate informal supports and formal services across agencies, departments and the communities in order to ensure services are integrated and responsive to the needs of you and your family. Staff provide leadership for the service team to make sure that your voice is heard, plans build on your strengths and abilities, and plans and goals from different providers are coordinated and communicated well. We help interventions and services to be delivered in a way to avoid crises.
Who is eligible? Families and individuals in Lamoille and Washington Counties referred through the Vermont Agency of Human Services.

Re-entry Program
We support women and men involved with Vermont's Department of Corrections to find and secure the life skills and perspectives needed to successfully re-enter the community after incarceration. Our staff builds a positive relationship with participants and their other service providers to assist with individual situations.
DETER (Drug Education, Treatment, Enforcement & Rehabilitation)
CVCAC's DETER project provides substance abuse and mental health assessments for women who are re-entering their communities following incarceration. Staff coordinate services and help participating women find support from peers.
